We are seeking a highly skilled Clinical Navigator to join our team at Hope Community Resources, Inc. As a Clinical Navigator, you will play a critical role in cultivating referral relationships, managing assigned territories, and completing patient assessments.

Key Responsibilities:
  • Develop and maintain strong relationships with referral sources, including physicians, discharge planners, and case managers.
  • Conduct on-site pre-admission assessments and provide clinical information to obtain pre-certification from payers.
  • Collaborate with the Admissions department and Director of Business Development to optimize census management and case finding strategies.
  • Provide education and support to patients, families, and referral sources on facility services and programs.
  • Stay up-to-date on new programs and services in the Post-Acute Medical System through frequent communication and facility visits.
Requirements:
  • Minimum of two years' experience in a healthcare field, preferably with rehabilitation and marketing experience.
  • Excellent clinical and assessment skills, effective presentation and training skills, and excellent written and oral communication skills.
  • Ability to collect, document, and analyze data, and knowledge of PPS payment/reimbursement systems and DRGs/case weights.
  • Knowledge of patient rights and commitment to patient advocacy.
