We are seeking a highly skilled Pharmacist-Medical Safety Clinical Specialty to join our team at Ascension Saint Agnes in Baltimore, Maryland. As a key member of our pharmacy team, you will be responsible for ensuring the safe and effective use of medications in our inpatient setting.
Responsibilities- Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, and cardiology.
- Provide guidance and support to clinical leadership and pharmacy staff on medication safety and use.
- Review and assess medication orders to identify, prevent, and resolve potential and actual drug-related problems.
- Recommend formulary alternatives when appropriate.
- Compound and dispense medications for accurate delivery to patients.
- Provide drug information and education to medical staff and patients.
- Communicate with physicians and other clinicians as necessary to facilitate care.
- Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies.
- Assist with academic functions, such as serving as a preceptor for pharmacy students.
- May float to provide coverage of pharmacy needs.
- Provide patient counseling when required.
- Pharmacist credentialed from the Maryland Board of Pharmacy obtained prior to hire date or job transfer date required.
- Bachelor's or advanced degree in Pharmacy required.
- Completion of Pharmacy Practice Residency training highly desirable.
- Working knowledge of Pyxis/Pyxis ES, Sentri 7, Meditech with sterile compounding, antimicrobial stewardship, and pharmacokinetic dosing experience.
- Ability to work independently and cooperatively with hospital and pharmacy staff to meet departmental and organizational goals.
- Advanced study in pharmacy or health care related field.
- Pharmacy residency or comparable work experience.
- Membership in ASHP and associated area of practice.
- Board Certification (BCPS or BCACP, BCOP).
- Hospital experience preferred.
- PGY1 or PGY2 Residency with focus towards specific practices, such as cardiology or ICU.
- Specialty pharmacy experience in a hospital setting.
- Knowledge of contemporary hospital/clinical practice and service.
