Job Description

We are seeking a skilled Hospice RN/Case Manager to join our team of compassionate caregivers. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans.

Responsibilities

  • Assess patient needs and create comprehensive care plans in collaboration with the healthcare team.
  • Provide direct patient care services, including wound care, pain management, and symptom control.
  • Coordinate with healthcare providers to ensure seamless transitions of care.
  • Maintain accurate and detailed records of patient interactions and care activities.
  • Participate in ongoing education and training to stay current on best practices in hospice care.

Requirements

  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
