Responsibilities- Provide one-on-one care to pediatric patients in their homes, promoting a safe and supportive environment.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure seamless care coordination.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Collaborate with healthcare professionals to ensure comprehensive care, including physical, emotional, and developmental support.
- Document patient progress, maintain accurate records, and communicate effectively with healthcare teams.
- RN/LPN with a current valid license to practice as issued by the State Board of Nursing.
- Current CPR certification.
