**Key Responsibilities:**
* Assess the healthcare needs of residents and develop individualized care plans to ensure the best possible outcomes.
* Administer skilled nursing care as prescribed by physicians and oversee the execution of treatment plans.
* Monitor and document resident's medical conditions, vital signs, and response to treatment, maintaining accurate and up-to-date records.
* Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to provide comprehensive care and support.
* Educate residents and their families on health conditions, medications, and self-care techniques to promote independence and well-being.
* Ensure compliance with all state regulations and facility policies, maintaining a safe and healthy environment for patients and staff.
* Utilize computer systems for accurate record-keeping and reporting, staying up-to-date with the latest technology and best practices.
* Participate in staff meetings and training programs to enhance clinical knowledge and skills, staying current with industry developments and advancements.
**Qualifications:**
* NYS RN license
* 2-4 years of clinical experience
* Computer literacy to effectively use electronic health records and other digital healthcare tools
