Overview

As a Full-time RN, you'll play a critical role in delivering high-quality care to our patients. You'll perform skilled nursing visits, coordinate client care, and oversee the frequency of visits for each episode. You'll also consult with physicians and office staff to ensure seamless care delivery.

Responsibilities
  • Perform skilled nursing visits and complete coordination of client care
  • Point of contact for all disciplines involved in patient care
  • Oversee the frequency of visits for each episode
  • Consult with physicians and office staff as needed
  • Accurately collect OASIS data and ensure medication profiles remain current
  • Report lab values to physicians in a timely manner
  • Attend weekly case conferences and monthly case manager meetings
Qualifications
  1. Currently licensed as a Registered Nurse (RN) in the state of employment
  2. A minimum of one year of clinical experience preferred
  3. Demonstrate knowledge and skill in current nursing practice
  4. Possess a valid state driver's license and automobile liability insurance
