As a Registered Nurse Case Manager, your responsibilities will encompass delivering nursing care aligned with the patient's care plan. This includes thorough health and psychological assessments, monitoring patient conditions, promoting health and prevention strategies, coordinating services, and providing education and training. We understand the importance of work-life balance, which is why this position does not require weekend on-call duties. Our innovative admission process is designed to reduce the OASIS and documentation workload for our RNs.

Working Hours: Monday - Friday, 8:00 AM to 4:30 PM

Service Areas: Mahoning and Trumbull counties.

Key Responsibilities:

  • Coordinate comprehensive patient care
  • Continuously assess the effectiveness of nursing services for patients and their families
  • Prepare and present patient records to the Clinical Record Review Committee as necessary
  • Submit clinical documentation regularly, including progress notes and service outlines
  • Engage in the patient discharge planning process
  • Perform other assigned duties as required

Qualifications:

  • Graduation from an accredited School of Nursing
  • Current Registered Nurse license in the State
  • Preferred: One or more years of experience in community/home health or hospital settings
  • Knowledge of Medicare and Medicaid regulations
  • Successful completion of a criminal background check
  • Current CPR certification

Job Conditions:

  • Valid driver's license and willingness to travel to patient residences
  • Ability to access homes that may not be wheelchair accessible
  • Physical capabilities to perform assessments and patient care, including lifting and turning patients

*This position offers a competitive hourly wage with opportunities for additional earnings through per diem visits.*
