Physiatrist Position

We are seeking a skilled Physical Therapist to join our team at HCA Virginia Healthcare at Home. As a Physical Therapist, you will play a vital role in providing high-quality patient care and supporting our patients throughout their recovery at home.
Job SummaryThe Physical Therapist will be responsible for managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team. This includes providing or directing a majority of the patient's physical therapy to support patients throughout the continuum of care.
Responsibilities- Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
- Develop and implement individualized plans of care for patients.
- Collaborate with the patient care team to ensure seamless care coordination.
- Provide education and support to patients and their families to promote optimal health outcomes.
A Bachelors Degree is required for this position. We are dedicated to providing patients with quality care during their recovery at home. Our team of professionals develops an individualized plan of care for each patient under the supervision of a physician.
