Key Responsibilities
  • Implementation of Physicians' Prescribed Plan of Care: You will work closely with our patients' physicians to implement their prescribed plan of care, ensuring that our patients receive the highest level of care possible.
  • Patient Assessments and Coordination of Care: You will conduct thorough assessments of our patients' needs and coordinate their care with our team of healthcare professionals.
  • Administration of Prescribed Medication, Treatments, and Therapies: You will administer prescribed medication, treatments, and therapies to our patients, ensuring their safety and well-being.
  • Compliant, Timely Documentation of Care Provided: You will maintain accurate and timely documentation of the care provided to our patients, ensuring compliance with regulatory requirements.
  • Health Promotion, Teaching, and Training of Family Members: You will educate and train our patients' family members on health promotion, disease prevention, and management of their loved one's condition.
  • Responsible for Accepting and Writing Verbal Orders from Physicians: You will accept and write verbal orders from physicians, ensuring that our patients receive the care they need in a timely manner.
Qualifications
  • Active, Unencumbered License (RN/LPN/LVN): You must have and maintain an active, unencumbered license in the state in which you will practice.
  • Current CPR Certification: You must have current CPR certification from the American Red Cross, American Heart Association, or BLS.
  • TB Skin Test (Current within Last 12 Months): You must have a current TB skin test within the last 12 months.
  • Six Months Prior Hands-On Nursing Experience Preferred: While not required, six months prior hands-on nursing experience is preferred.
  • Reliable Transportation: You must have reliable transportation to travel to our patients' homes.
