Sentara Health is looking for a dedicated healthcare professional to become part of our office-based Comprehensive Care team. We are focused on delivering an exceptional patient experience in a supportive environment.
Key Responsibilities:
- Work Schedule: 8:00 AM - 5:00 PM, 4 days per week dedicated to patient care, with one half-day allocated for administrative duties (the remainder of the day off).
- Deliver comprehensive and primary care to patients with chronic conditions in an office environment.
- Manage an average caseload of 16 patients daily.
- Concentrate on minimizing hospital readmission rates through proactive patient education and management.
- Perform detailed assessments, diagnose health issues, and prescribe suitable treatments.
- Participate in a comprehensive onboarding program that includes a structured curriculum, specialty shadowing, competency evaluations, and mentorship.
