We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will be responsible for planning and administering medically prescribed physical therapy treatment programs in the home settings for patients to restore function, relieve pain and prevent disability following disease, injury or loss of body parts.
Key Responsibilities- Consult with physicians to develop treatment plans, including specific procedures and equipment to be used and frequency of visits.
- Administer physical therapy treatment programs to patients in their homes, focusing on restoring function, relieving pain, and preventing disability.
- Collaborate with other healthcare professionals to ensure comprehensive care for patients.
- Document patient progress and treatment plans in accordance with regulatory requirements.
- Master's degree in Physical Therapy from an accredited program.
- Current Washington Physical Therapist License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- National Provider Identifier (NPI) number within 30 days of hire.
- 1 year of experience in Home Health.
- 2 years of experience in Physical Therapy with adults with physical disabilities.
- 1 year of experience in the geriatric population.
