Job Summary
We are seeking an experienced Care Coordinator RN to join our team. The successful candidate will work collaboratively with patients, families, and healthcare providers to coordinate high-quality care that addresses the unique needs of each individual. This role requires strong communication skills, empathy, and a passion for delivering patient-centered care.

Responsibilities
* Systematically identify patients in need of coordinated care and develop personalized plans to address their specific needs.
* Collaborate with patients, families, and healthcare providers to ensure seamless transitions of care.
* Provide education and support to patients and their families to promote healthy behaviors and disease management.
* Coordinate referrals to community resources and services as needed.

Requirements
* 3-5 years of experience in clinical or community resource settings.
* Completion of an accredited Registered Nurse training program.
* Certification through the American Academy of Ambulatory Care Nursing in Care Coordination and Care Transitions within 18 months of employment.
