Senior Nurse Practitioner

Job Summary:

UnitedHealth Group is seeking a Senior Nurse Practitioner to join our team in Home and Community Care. As a key member of our team, you will provide high-quality, patient-centered care to seniors in their homes and communities.

  • Key Responsibilities:
  • Deliver cost-effective, quality care to assigned members
  • Manage both medical and behavioral, chronic and acute conditions effectively
  • Perform comprehensive assessments and document findings in a concise/comprehensive manner
  • Responsible for ensuring accurate ICD10 coding and documentation

Requirements:

Certified Nurse Practitioner through a national board

Graduate of an accredited master's degree in Nursing (MSN) program

Active and unrestricted APC license in the State of New Jersey, or the ability to obtain by start date

Ability to lift a 30-pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations

Preferred Qualifications:

1+ years of hands-on post-grad experience within Long Term Care

Understanding of Geriatrics and Chronic Illness

Understanding of Advanced Illness and end of life discussions
