Responsibilities- Conduct thorough patient assessments and develop individualized care plans to address patient needs
- Collaborate with the healthcare team to implement evidence-based interventions and promote optimal patient outcomes
- Provide education and support to patients and their families to ensure a smooth transition to home or further care
- Participate in quality improvement initiatives to enhance patient care and safety
- Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program
- Current state licensure as a Registered Nurse
- CPR/BCLS certification; ACLS preferred
- Minimum six months' experience in an acute care setting, preferably in medical rehabilitation
- Excellent communication and interpersonal skills
