Responsibilities- Conduct thorough assessments of patients' physical, emotional, and social needs, developing and implementing individualized care plans to address these needs.
- Provide skilled nursing care, including wound care, medication management, and vital sign monitoring, as ordered by the patient's physician.
- Collaborate with the healthcare team to coordinate care and services, ensuring seamless transitions and optimal patient outcomes.
- Document patient information accurately and thoroughly, maintaining up-to-date records and communicating effectively with the healthcare team.
- Comply with Home Health policies and procedures, adhering to infection control guidelines and maintaining a safe and healthy work environment.
- Current LPN licensure in the state of Pennsylvania.
- Experience in home health care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, prioritizing tasks and managing time efficiently.
- Basic computer skills and proficiency in electronic medical records.
- Competitive hourly rate: $38.00 per hour.
