We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Dodge County. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.
Responsibilities- Assist and participate in the development of total care plans for residents, working collaboratively with physicians, psychiatrists, nurses, and allied therapists.
- Provide and document nursing care for residents, observing and reporting on their physical and mental condition, and obtaining vital signs.
- Document nursing interventions, individual responses, effectiveness of treatments, complications, and other relevant information.
- Participate in data collection from individuals upon admission, identifying problems, and assisting in developing initial treatment/care plans.
- Update treatment/care plans and assessment forms as necessary, under the direction of an RN and in conjunction with the interdisciplinary team.
- Provide leadership and direct functions of Household Assistants, assisting with admissions, discharges, and transfers, and notifying RN of any change in resident condition.
- Administer medications and treatments as prescribed by physician and psychiatrist, and provide support and information to residents or family members as needed.
- Perform housekeeping and laundry responsibilities as needed, and assist with the training and orientation of new Team Leads.
- Knowledge of the principles, standards, practices, and techniques of Skilled Nursing.
- Knowledge of the dangers inherent in and the precautions to be taken in the use of drugs and other medication.
- Knowledge of local and state rules, regulations, procedures, and policies pertaining to nursing services.
- Ability to assign and supervise the work of professional and nonprofessional personnel, and to instruct them in practices and techniques.
- Ability to accurately maintain patient medical records and prepare reports.
- Knowledge of electronic records systems.
- Knowledge of PPE and its proper use for personal safety measures.
- Ability to maintain confidentiality of all resident care information and assure resident rights and privacy are protected at all times per HIPAA.
- Knowledge of chemicals used in housekeeping and laundry services, including hazardous laundry chemicals and isolation procedures.
- Ability to create and maintain an atmosphere of warmth and personal interest, as well as a clean environment.
- Ability to work tactfully and cooperatively with residents, families, visitors, and entire staff throughout the organization.
- Ability to verbally communicate with residents, families, the public, and other members of the organization.
- Must be able to exhibit a warm, cheerful, caring manner.
- Must be able to safely perform the essential job functions.
- Ability to interpret and implement local policies and procedures, written instructions, general correspondence, Federal, State, and local regulations.
- Skill in organizational and time management to prioritize duties to accomplish a high volume of work product while adapting to constant changes in priority.
- Ability to perform detailed work accurately and independently in compliance with stringent time limits, requiring minimal direction and supervision.
- Ability to think quickly, maintain self-control, and adapt to stressful situations.

- Ability to prepare and maintain accurate and concise records and reports.

This role requires occasional exertion of up to 75 pounds of force, and regular sitting, frequent standing, speaking or hearing, using hands to finger, handle or feel, and repetitive motions. Work has standard vision requirements, and vocal communication is required for expressing or exchanging ideas by means of the spoken word. Hearing is required to perceive information in moderately loud conditions. Work frequently requires exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment, and occasionally requires exposure to fumes or airborne particles.
