Position Summary:

PCG INTERNATIONAL INC is seeking an Epic Care Healthy Planet Specialist who will play a crucial role in the deployment, configuration, and ongoing support of the Epic Care Healthy Planet module within a healthcare setting. This position requires collaboration with various stakeholders, including teams focused on population health management, quality enhancement professionals, and data analysts, to ensure that the Healthy Planet module is effectively integrated with the organization's strategies and objectives for population health management.

Core Responsibilities:

  • Configuration of Epic Care Healthy Planet Module: Tailor and sustain the Epic Care Healthy Planet module according to the organization's requirements for population health management, encompassing risk stratification models, workflows for care management, quality metrics, and patient outreach initiatives.
  • Data Integration and Analysis: Work alongside internal and external data sources to guarantee precise and timely integration of clinical, claims, and demographic data into the Healthy Planet module. Employ population health analytics tools to identify at-risk groups, care gaps, and potential areas for quality enhancement.
  • Workflow Evaluation and Enhancement: Partner with population health management teams to assess current workflows and pinpoint opportunities for streamlining care coordination, patient engagement, and population health initiatives. Implement modifications to boost efficiency and outcomes.
  • Reporting and Data Visualization: Create and sustain detailed reports, dashboards, and data visualizations within the Healthy Planet module to support population health management efforts. Provide actionable insights and data-driven recommendations to stakeholders.
  • Training and User Support: Design and deliver training sessions to inform population health management teams, quality improvement professionals, and other users on utilizing the Healthy Planet module. Offer ongoing assistance, troubleshoot issues, and address user inquiries promptly.
  • Compliance with Regulations: Remain informed about relevant regulatory guidelines and requirements concerning population health management, quality reporting, and value-based care initiatives. Ensure that the Healthy Planet module adheres to these standards and assist in the documentation and reporting of quality metrics.
  • Documentation and Project Oversight: Maintain thorough documentation of workflows, system configurations, and modifications made to the Healthy Planet module. Oversee projects related to module enhancements, upgrades, and new implementations, ensuring compliance with timelines and deliverables.
  • Commitment to Continuous Improvement: Stay updated on industry trends and best practices related to population health management, value-based care, and electronic health record systems. Recommend enhancements to the Healthy Planet module to improve analytics capabilities, care coordination, and patient outcomes.

Qualifications:

  • Bachelor's degree in healthcare informatics, public health, healthcare administration, or a related discipline. Relevant certifications in Epic Care Healthy Planet are highly preferred.
  • Experience with the Epic EHR system, particularly in configuring and optimizing the Healthy Planet module.
  • Strong grasp of population health management principles, including risk stratification, care management, quality improvement, and value-based care.
  • Familiarity with healthcare data sources, including clinical, claims, and demographic data, and experience in integrating data into population health management systems.
  • Proficiency in population health analytics tools and methodologies, such as data modeling, visualization, and performance reporting.
  • Exceptional analytical and problem-solving abilities, with a talent for interpreting complex data and deriving actionable insights.
  • Strong communication and interpersonal skills for effective collaboration with cross-functional teams, stakeholders, and end-users.
