Physical Therapist for Home Health

FeldCare Connects is seeking a motivated and compassionate Physical Therapist to deliver high-quality home healthcare to patients of all ages in Oxnard and surrounding areas.
Key Responsibilities- Provide one-on-one treatment to patients in their homes, observing, recording, and reporting their response to treatment and changes to their condition.
- Determine the plan of care for each patient, working independently as an independent contractor.
- Utilize FeldCare Connects' Electronic Medical Record system for easy documentation, completing records during visits when possible.
- Access patients through FeldCare Connects' innovative app Cliniconnects, accepting or declining patients in seconds.
- Current Physical Therapist license issued by the state Board of Physical Therapy, unrestricted and active.
- Completion of an accredited Physical Therapy program.
- Reliable personal transportation and a smartphone.
- Bilingual a plus.
FeldCare Connects addresses the industry-wide challenge of finding care for homebound patients by connecting clinicians to patients in their community. By joining the FeldCare network, you will give patients access to care they would otherwise go without.
We connect over 2,000 clinicians to patients in their community by contracting with over 900 home health agencies throughout CA, AZ, TX, and FL. By creating these links, we allow home health agencies and hospices to give their patients access to the care they need.
By taking the administrative burden off agencies and clinicians, they can focus more of their attention on patients, resulting in better patient outcomes.
