About the Role

As a Licensed Practical Nurse LPN, you will be responsible for:

  • Providing direct patient care, including administering medications, performing routine procedures, and monitoring patient conditions.
  • Assessing patient needs and developing individualized care plans to promote optimal health outcomes.
  • Collaborating with healthcare teams to ensure seamless care coordination and communication.
  • Documenting patient information and maintaining accurate records.
Requirements

To be successful in this role, you will need:

  • A valid license as a Registered Nurse in the state of Michigan.
  • Physical within the past six (6) months.
  • Certified CPR and negative TB skin test or chest x-ray.
  • Minimum one (1) year of documented experience in a home health agency, primary care clinic, or health facility.
