Job Summary:

The Licensed Vocational Nurse (LVN) is responsible for assisting with the coordination of home care services to ensure the health and safety needs of residents are met. The LVN will work closely with the team to maintain compliance with California regulations and ensure residents maximize their independence.

Key Responsibilities:
  • Administer medications to individuals and verify the five rights for accuracy
  • Attend residents' doctor appointments and address any concerns or changes with the attending physician
  • Assist individuals with activities of daily living, such as bathing, grooming, and preparing meals
  • Audit medications during new medication cycles and verify the five rights
  • Perform g-tube insertions, colostomy care, and intramuscular injections as needed
  • Evaluate residents' level of consciousness, neurological factors, and responsiveness post-seizure or traumatic episodes
  • Redirect and support individuals during behavioral episodes
  • Complete and update client notes system daily, observing and reporting any unusual behaviors or injuries
  • Assess and identify signs and symptoms of infections, performing basic physical reviews and head-to-toe assessments
  • Participate in community outings and leisure activities with residents
Requirements:
  • 1 year of experience supporting adults with developmental disabilities
  • Licensed Vocational Nurse License
  • First Aid/CPR and CPI certification
  • 21 years of age or older
  • California Driver's License
  • Pass all pre-employment requirements, including background checks
