The OpportunityWe are seeking a skilled Nurse Practitioner to join our team in the Bay Area. As a key member of our primary care team, you will be responsible for delivering exceptional care to our patients, working closely with our care navigators to maximize the time and energy of our providers.
Key Responsibilities:- Provide onsite primary care services to older adults living in senior living communities
- Develop and execute detailed person-centered care plans for all primary care patients
- Collaborate with our Medical Director to identify and develop policies addressing emerging issues and clinical guidelines
- Work with Primary Care Coordinators to provide monthly Chronic Care Management visits and care
- Support diagnostic needs for each patient in communities through collaboration with Traveling Clinical Assistants
- Lead regular huddles and provide clinical leadership to the team
- Identify opportunities for collaboration with senior living management and care teams to enhance patient outcomes
- Board-Eligible or Board-Certified Physicians in Internal Medicine or Family Medicine, or Nurse Practitioner or Physician Assistant with geriatrics experience
- Comfortable working in a fast-paced, dynamic environment
- Ability to provide onsite clinical care across the Bay Area and drive between partner communities as needed
- Strong orientation towards collaboration and working closely with a multidisciplinary team
- Excellent written and verbal communication skills
- Ability to work with tech-enabled care delivery tools

Please note that significant travel and startup hours are required for this role. You must have the ability to drive between partner communities as needed. Pine Park Health requires vaccination for all employees and proof of vaccination before hiring.
The salary range for this role is $140,000 to $180,000, inclusive of base, annual performance bonus, sign-on bonus, plus equity and benefits. The final compensation package will depend on several job-related factors unique to each candidate.
