Job Description
We are seeking a skilled Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our team in providing home-based nursing care to pediatric and adult patients. As an LPN/LVN, you will be responsible for assessing the needs of each patient, developing individualized care plans, and implementing interventions to promote optimal health outcomes.

Key Responsibilities:
  • Utilize the nursing process to assess, plan, implement, and evaluate patient care.
  • Provide skilled nursing care/services in accordance with prescribed orders.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.

Requirements
