- Train and support new and existing team members to ensure effective teamwork and knowledge sharing.
- Provide backup support to the Medical Assistant team, covering all aspects of medical and administrative duties.
- Participate in interdisciplinary teams to evaluate and assess patient needs, coordinating care with primary physicians and other healthcare providers.
- Observe patient behavior patterns and medical symptoms, reporting findings to prescribing clinicians or clinical staff for follow-up.
- Perform nursing screenings and assessments, reporting findings to prescribers.
- Deliver integrated care services to patients, collaborating with clinical and other staff to address patient needs.
- Implement and revise integrated service plans, ensuring services align with patient goals and monitoring progress.
- Support patients in following prescribed medical interventions and diagnostic testing, educating them on test results and prescribed treatments.
- Assess and address barriers to health, facilitating communication with patients and healthcare providers as needed.
The ideal candidate will possess excellent communication and leadership skills, with the ability to work effectively in a fast-paced clinical environment. A strong understanding of medical terminology and procedures is essential, as is the ability to maintain confidentiality and handle sensitive patient information.
