Job Summary:

Under the direction of a Registered Nurse, the Licensed Practical Nurse will deliver efficient and effective nursing care to patients in a skilled nursing facility. The ideal candidate will have a strong background in rehabilitation nursing and be able to work effectively in a team environment.

Responsibilities:
  • Collects, reports, and documents objective and subjective data to inform patient care.
  • Observes patient conditions and reports changes to the Registered Nurse.
  • Contributes to establishing individualized patient goals and develops interventions to achieve them.
  • Implements the plan of care and evaluates its effectiveness in achieving patient goals.
  • Participates in review and revision of the plan of care.
  • Administers medications and performs treatments per physician orders.
  • Communicates pertinent data to the Registered Nurse and/or physician.
  • Documents accurately and thoroughly.
  • Consults and seeks guidance from the Registered Nurse as necessary.
Requirements:
  • Graduate of an approved school for practical or vocational nursing.
  • Currently licensed by the State Board of Nursing.
  • Experience in rehabilitative or geriatric nursing is preferred.
  • Ability to read, write, speak, and understand the spoken English language.
  • Verification of TST (tuberculin skin test) as required by state law and in accordance with Company policy.
  • Current BLS/CPR certification.
Work Environment:

The Licensed Practical Nurse will work in a skilled nursing facility, providing care to patients in a rehabilitation setting. The ideal candidate will be able to work effectively in a fast-paced environment and prioritize tasks to meet the needs of patients.
