Job Summary:

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our adult and geriatric clients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing clients' physical abilities, developing and implementing individualized care plans, and educating clients and their families on rehabilitative care and activities.

Responsibilities:
  • Conduct home visits to assess clients' physical abilities and develop personalized care plans.
  • Implement manual therapeutic exercises, gait training, balance, and other interventions to promote clients' health and safety.
  • Educate clients and their families on rehabilitative care and activities to promote independent living.
  • Accurately document observations, interventions, and evaluations in a state-of-the-art touch pad tablet.
  • Collaborate with nursing and other healthcare professionals to develop and implement comprehensive care plans.
Requirements:
  • A current Maryland Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
