About the Role:
Prestige Care Center of Fort Collins is seeking a highly skilled and detail-oriented individual to fill the position of Resident Assessment Coordinator and Infection Preventionist. As a key member of our team, you will be responsible for ensuring the highest quality of care for our residents while promoting a culture of excellence and empowerment.
Key Responsibilities:
- Resident Assessment Instrument (RAI) Process Coordination: Oversee the facility's RAI process in accordance with state and federal regulations, ensuring accurate and timely completion of all MDS assessments and supporting documentation.
- Comprehensive Plan of Care Implementation: Develop and implement individualized care plans that address the unique needs of each resident, working closely with the interdisciplinary care team to ensure seamless execution.
- Infection Prevention and Control: Identify, manage, and treat skin conditions, such as pressure injuries, diabetic, venous, or arterial ulcers, and traumatic or complicated wounds, providing education to residents, families, and staff on infection prevention and control principles.
- Wound Assessment and Management: Conduct thorough and accurate wound assessments, providing follow-up assessments as needed, and educating residents and their representatives on wound healing and preventive measures.
- Interdisciplinary Team Collaboration: Facilitate the completion and updating of resident care plans, communicate with the IDT to identify residents in need of a Significant Change in Status assessment, and ensure timely completion of MDS sections and CAAs.
- Quality Assurance and Compliance: Correct fatal errors immediately, retransmit assessments until an accepted validation report is received, and maintain accurate and up-to-date records.
Requirements:
- Current, unencumbered, active license to practice as a registered nurse in this state.
- 3 years' experience as a registered nurse, including supervisory experience, with experience in MDS completion preferred.
- Resident Assessment Coordinator (RAC) training completed within six months of hire, with RAC Certification preferred.
