Job Summary:We are looking for a dedicated and experienced RN to provide high-quality patient care to our adult and geriatric clients in their homes. As a per diem home health nurse, you will have the opportunity to work on a flexible schedule, making a positive impact on the lives of our clients and their families.
Responsibilities:- Provide skilled nursing care to clients in their homes, following a designated care plan and collaborating with the interdisciplinary team.
- Conduct thorough assessments of clients' physical and emotional needs, identifying areas for improvement and developing effective care plans.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Monitor clients' conditions, reporting changes to the Clinical or Client Services Manager.
- Document all interactions, interventions, and evaluations in a timely and accurate manner.
- A current license as a Registered Nurse in New Jersey.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred, but not required.
- Graduation from an accredited and approved nursing program.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and ability to communicate effectively with clients, families, and healthcare team members.
