Key Responsibilities
  • Client Services
    • Provide population health services to assigned clients on an ongoing and emergency basis, adhering to contracted intervention protocols and timelines.
    • Develop and maintain accurate and thorough knowledge of all program requirements, including HIPAA and GroundGame Health's confidentiality policies.
  • Case Management
    • Develop and maintain an accurate and thorough knowledge of community resources available to assist clients, working with other departments to acquire knowledge of community resources.
    • Organize and document daily schedule to inform supervisor of activities, adhering to safety protocols.
  • Communication and Collaboration
    • Communicate with supervisor on a daily basis, and as needed, to report all areas of concern, issues, or incidents to the State Account Manager.
    • Demonstrate professionalism in relationships with customers, coworkers, and supervisors, building rapport while refraining from inappropriately familiar personal relationships.
  • Administrative Tasks
    • Complete and attest to accuracy of payroll records and submit for review and processing according to GroundGame Health's schedule.
Requirements
  • Education
    • Associates Degree in social work, or related field, or High school diploma, or general education degree (GED); and four years of related experience and/or training in healthcare and/or customer service position utilizing advanced technology tools, including customized software systems.
  • Experience
    • Two years of experience and/or additional education in social work or related field, knowledge of medical terminology, and/or experience in a customer service position preferably in a healthcare setting or Certified as a Community Health Worker.
  • Additional Eligibility Requirements
    • Must possess and provide annually valid driver's license and documentation of current automobile insurance at agency-specified levels.
    • Must be available to work 5:00 pm - 8:00 pm and some weekends.
