As a skilled Home Health Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive admission, case management, and follow-up skilled nursing visits to home health patients.
  • Collaborate with physicians to develop and implement individualized care plans, ensuring seamless coordination of care.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies to promote optimal outcomes.
  • Coordinate with interdisciplinary teams, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions and address any concerns.
  • Document patient progress, update care plans, and maintain accurate records to ensure continuity of care.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in med-surg, ICU, ER, or acute care settings
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational, communication, and interpersonal skills
  • Valid driver's license, auto insurance, and reliable transportation
