Job Overview

The Physical Therapist at Applied Palliative and Hospice Services, Inc. plays a crucial role in delivering high-quality rehabilitation services. This position is essential for ensuring compliance with all stipulations outlined in the Personnel Services Agreement.


Key Responsibilities:
  • Deliver physical therapy services to patients based on a comprehensive plan established by a licensed healthcare provider.
  • Conduct thorough assessments to evaluate the therapeutic and functional status of patients, contributing to the overall care plan.
  • Evaluate the home environment to ensure safety and accessibility for rehabilitation.
  • Assess muscle strength, mobility, gait, and range of motion to determine rehabilitation potential.
  • Direct and implement physical therapy treatments tailored to individual patient needs.
  • Educate patients and their families on the use and maintenance of therapeutic devices.
  • Identify priority needs for physical therapy and communicate effectively with the healthcare team regarding patient progress and changes in condition.

Additional Duties:
  • Initiate and oversee physical therapy programs, guiding family members and caregivers in their involvement.
  • Provide training for patients on the use of prosthetic devices as necessary.
  • Recognize the need for additional home health services and facilitate referrals.
  • Prepare and submit clinical documentation and progress reports reflecting patient achievements.
  • Engage in discharge planning and collaborate with the healthcare team to ensure smooth transitions.
  • Offer physical therapy consultations to families as needed.
  • Conduct educational sessions for nursing staff to enhance their understanding of physical therapy practices.
  • Participate in peer review processes to maintain high standards of care.
  • Supervise Physical Therapy Assistants in accordance with organizational policies and state regulations.
  • When applicable, oversee home health aide services bi-weekly to ensure quality care.

Qualifications:
  • A degree from an accredited baccalaureate or master's program in physical therapy.
  • Current licensure to practice as a physical therapist in the relevant state.
  • A minimum of two years of experience in physical therapy, preferably in community or home health settings.
  • Strong verbal and written communication skills, along with excellent organizational abilities.
  • Current CPR certification is required.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
