Sunrise Senior Living is seeking a compassionate and dedicated Care Manager to join our team at Sunrise of Lenexa. As a Care Manager, you will play a vital role in providing high-quality care and services to our residents and their families.

Responsibilities:
  • Build meaningful relationships with residents and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Notify management of changes in condition and recommend adjustments in the level of care and service.
  • Assist residents in life skills and other life-enriching activities, blending a variety of multisensory experiences into their daily routine.
  • Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
  • Responsible for a designated group of residents during shifts, ensuring their safety and well-being.
  • Partner with the community team to maintain a safe and healthy environment, adhering to OSHA requirements and promoting Risk Management programs and policies.
  • Respond to dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
Qualifications:
  • Dedication to serving seniors with excellent customer service skills.
  • Positive attitude, flexibility, and ability to work in a team environment.
  • High School diploma or equivalent, with certifications and licenses as required by state regulations.
  • Previous experience working with seniors preferred.
  • Ability to make choices, decisions, and act in the resident's best interest.
  • Competent in organizational and time management skills.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
