Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in York County. As an LPN, you will play a vital role in delivering high-quality patient care while working under the direction of a Registered Nurse (RN).

Key Responsibilities
  • Patient Evaluation: Conduct thorough assessments of patients to identify their needs and develop effective care plans.
  • Care Planning: Collaborate with the RN to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provision of Direct Patient Care: Provide hands-on care to patients, including administering medications, performing treatments, and monitoring vital signs.
  • Supervision of Unlicensed Staff: Supervise and delegate tasks to unlicensed staff members to ensure that patient care is delivered efficiently and effectively.
  • Point Click Care (PCC) Utilization: Use PCC according to the Business Process to ensure accurate and timely documentation of patient care.
  • Medication Management: Perform drug counts at the beginning and end of each shift, and as needed, to ensure that medications are properly managed.
  • Collaboration with Physicians: Assist physicians in rounds and examinations as needed to ensure that patients receive comprehensive care.
  • Shift-to-Shift Communication: Participate in shift-to-shift communication with incoming and outgoing nursing staff to ensure a smooth transition of care.
  • Interdisciplinary Collaboration: Collaborate with other departments to provide timely and effective care that meets the individual needs of each patient.
  • Culture of Safety: Contribute to a culture of safety that promotes a healthy practice and living environment.
  • Confidentiality and HIPAA: Maintain confidentiality and protect sensitive Protected Health Information (PHI) at all times.
Requirements
  • Education: Graduate of an approved school for practical or vocational nursing.
  • Licensure: Currently licensed by the State Board of Nursing.
  • Experience: Experience in rehabilitative or geriatric nursing is preferred.
  • Certifications: Maintains current BLS/CPR certification.
