Job Summary

The RN Home Health Specialist will be part of a team creating a multi-dimensional care experience for our patients. Responsibilities
  • Practice professional nursing principles, concepts, and procedures as applied to the care of the patient population served.
  • Record patients' medical history and symptoms to help develop an appropriate plan of care to obtain the patients' optimal functional goal.
  • Perform diagnostic tests and analyze results, operate medical equipment, administer treatment and medications, and help with patient follow-up.
  • Establish a care plan or contribute to an existing plan, including activities such as administering and teaching patients/patient caregiver medication administration, careful medication management, starting, maintaining, and discontinuing peripheral intravenous (IV) lines, peripheral and central venous access devices for fluids, nutrition, and medication administration.
  • Administer therapies and treatment, observe the patient and record those observations, and consult with physicians and other healthcare clinicians.
  • Teach patients and their families how to manage their illnesses or injuries, explain post-treatment and home care needs, diet, nutrition, and exercise programs, and self-administration of medication and therapy.
  • Provide direction to nursing aides regarding patient care and delegate when appropriate.
  • Support efforts to advance clinical knowledge and skills.
  • Precept new hire staff and students.
  • Assist the staff and students to seek and complete the tasks required to complete their competency skills.
  • Continuously enhance knowledge in nursing management and care of patient problems and conditions as required.
Qualifications
  • Associate's Degree or Diploma in Nursing.
  • 2 years RN experience.
  • Registered Nurse (RN) State And/Or Compact State Licensure.
  • Basic Life Support (BLS) Certification in BLS training course C, and unit specific certification as required by specialty area.
  • Fingerprint Clearance Card - State Current Arizona Dept. of Health Services Fingerprint Clearance Card or shall apply and be eligible for a fingerprint clearance card within twenty (20) working days of employment.
