As a Registered Nurse - Diabetes Educator, you will play a vital role in providing high-quality care to patients with diabetes. This is a challenging and rewarding position that requires strong clinical skills, excellent communication, and a passion for patient education.
Responsibilities- Develop and implement individualized diabetes education plans for patients, taking into account their unique needs and goals.
- Provide comprehensive nutrition care plans for patients, including calorie and nutrient intake recommendations, and ensure compliance with established care plans.
- Assist patients in identifying and setting personal goals for improvement, and provide frequent follow-up support to ensure goal success.
- Identify and address barriers to patient care goals, such as financial or physical barriers, and work with patients to resolve these issues.
- Chart patient progress in the electronic medical record, including diabetes education and patient updates.
- Follow up with high-risk patients to ensure timely and continuous support, and notify physicians and health coaches of any pertinent patient issues.
- Collaborate with physicians and health coaches to provide quality care to patients with diabetes, and participate in interdisciplinary activities to ensure adequate communication and patient care planning.
- Bachelor's degree in Nursing (BSN) from an accredited school of nursing.
- Three years of clinical nursing experience, with a minimum of two years of experience in diabetes education.
- Current valid Registered Nurse (RN) license, valid compact multi-state license, or a temporary permit while awaiting licensure.
- Certification in area of specialty preferred, such as Certified Diabetes Care and Education Specialist (CDCES).
