We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Medication Care Manager at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care and services for our residents by administering medication and treatments in a safe and organized manner.
Responsibilities- Administer medications and treatments to residents in accordance with their individualized care plans and physician orders.
- Conduct medication counts and audits to ensure accuracy and prevent medication errors.
- Document all medication administration, including any refusals or missed doses, in the resident's medical record.
- Collaborate with the Resident Care Director and Wellness Nurse to ensure accurate and timely medication updates.
- Participate in daily Cross Over meetings to discuss resident care and medication management.
- Conduct Service and Health Updates as directed by the Resident Care Director.
- Transcribe orders and maintain accurate and up-to-date resident records.
- Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
- Ensure compliance with all regulatory requirements, including OSHA and British Columbia Workers Compensation Act.
- Participate in ongoing training and education to maintain certification and stay up-to-date on best practices in medication management.
- Valid Ohio LPN license.
- CPR and First Aid certification.
- Ability to make choices and decisions in the best interest of residents.
- Ability to react and remain calm in difficult situations.
- Ability to handle multiple priorities and work in a fast-paced environment.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
