**Job Description**
We are seeking a highly motivated Registered Nurse to join our team as a RN Case Manager. In this role, you will be responsible for managing patient care plans, coordinating treatment activities, and communicating with our interdisciplinary team members.
The successful candidate will have excellent communication and interpersonal skills, with the ability to work independently and as part of a team. They will also possess strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
Responsibilities:
- Develops a care plan in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission.
- Cordinates the patient's care plan through interdisciplinary communication and case conferences, as appropriate.
- Provides ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.
- Documents evaluations, treatment goals, and objectives.
Qualifications:
- A licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing.
