Key Responsibilities- Assist the Team Leader in overseeing the day-to-day operations of the Intensive Mobile Treatment (IMT) team.
- Work collaboratively with clients, families, and significant others to develop and implement person-centered treatment plans.
- Conduct comprehensive assessments and screenings to identify clients' needs and develop strategies for engagement and treatment.
- Advocate and liaison for clients within the criminal justice and shelter systems to ensure seamless transitions and access to necessary resources.
- Provide or connect clients with employment counseling, vocational rehabilitation, housing support, and life skills training to promote integrated physical and mental health, safety, wellbeing, and recovery.
- Deliver individual counseling sessions that incorporate principles of Integrated Dual Disorder Treatment, as appropriate.
- Maintain accurate and timely progress notes and assessments in the electronic health record.
- Collaborate with external stakeholders, such as hospitals, shelters, and primary care providers, to achieve positive client outcomes.
- Provide services in a strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.
- Master's Degree in social work, counseling, or a related field.
- Active New York State license in Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC).
- At least two years of experience working with adults who have needs in the areas of mental health, substance use, trauma, violence risk, homelessness, frequent psychiatric hospitalizations, and criminal-justice involvement.
- Prior supervisory experience is desired.
- Strong commitment to social justice and the CASES mission.
- Experience in a multidisciplinary setting and field-based clinical work.
- Previous use of an electronic health record, Footholds AWARDS.
- Fluency in Spanish.
