Key Responsibilities:

  • Provide clinical services to patients in their homes, including assessments, care planning, and implementation.
  • Develop and revise individualized plans of care in collaboration with other healthcare professionals.
  • Ensure patient eligibility and medical necessity for services, as defined by payer source and agency policy.
  • Collaborate with patients, families, and other community resources to achieve and maintain continuity of care.

Requirements:

  • Current RN licensure in the state of practice.
  • Minimum of one year experience as an RN, with recent clinical experience as an LPN considered in some states.
  • Current CPR certification.
