Home Health Nurse Specialist

We are seeking a skilled Registered Nurse to provide home health services in the Home Health Certified department. The successful candidate will work as part of a team to deliver high-quality patient care and follow established guidelines for clinical practice.
About the Role:
- This is a 13-week local contract position with day shifts (08:00-16:30 Hours) from Monday to Friday, covering approximately 40 hours per week.
Key Responsibilities:
- Analyze clinical data to diagnose nursing problems and identify nursing diagnoses, utilizing evidence-based practices.
- Provide individualized care to clients with complex needs, including pediatrics, geriatrics, and mental health patients, emphasizing patient-centered care and autonomy.
- Critical thinking skills are essential in the home environment to identify situations requiring physician notification or emergent care.
- Care for clients with various diagnoses, including orthopedic, surgical, medical, cardiac, wound/ostomy, and home infusion needs, adhering to current standards of care.
- Deliver health coaching and education to patients, families, and populations to achieve lower costs, engaged patients, and reduced hospitalizations and ER/Urgent visit usage.
- Involve the client/caregiver in goal development and plan of care development, ensuring a collaborative approach.
- Educate client/caregiver on disease process, treatment plan, and methods to increase independence for discharge planning, promoting self-care and empowerment.
- Confer with other disciplines to provide an interdisciplinary approach to meet all identified client care needs.
- Assess existing and potential problem areas, collecting data, and reporting significant information to the Clinical Manager or Clinical Leader.
- Function purposefully in emergency situations, demonstrating sound judgment and decisive action.
- Instruct and supervise home health aides, LPNs, or other services provided to the client in their home requiring RN supervision.
- Maintain accurate, current, and complete documentation according to policy on a timely basis.
- Perform other responsibilities as requested by leadership, showcasing adaptability and a commitment to excellence.
Requirements:
- BLS and ACLS certification.
- Sound knowledge of anatomy, physiology, pathology, and medical terminology.
- The ability to stand for extended periods.
- Strong technical skills.
- Knowledge of medical terminology.
- Passionate about patient care.
- Requires a minimum of two years of current RN Home Health experience.
- Graduate from a fully accredited RN program.
- Active unencumbered Iowa RN state license.
- One year of Cerner experience.
