Key Responsibilities:

  • Provide lymphedema care and management to patients in their homes
  • Collaborate with healthcare professionals to develop and implement individualized care plans
  • Conduct thorough assessments and evaluations to identify patient needs
  • Develop and implement effective treatment plans to promote patient independence and well-being
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination

Requirements:

  • Current Michigan Registered Nurse (RN) licensure
  • Minimum one year of Home Health nursing experience (preferred but not required)
  • CLT Certificate or CLT LANA Credential from the Lymphology Assoc of North America (LANA)
  • Prior work experience with lymphedema nursing and working with interdisciplinary healthcare teams
  • Demonstrated knowledge and skills to provide competent, compassionate care to all ages of the population
  • Strong communication skills, both verbal and written
  • Updated Tuberculin (TB) test and current CPR certification required
  • Clear Criminal History/Drug Screen
  • Compassionate and caring demeanor, excellent communication skills, and ability to think critically
  • Ability to travel within the service area of AdvisaCare as needed
