Position Overview:
A seasoned clinical pharmacist responsible for delivering advanced pharmaceutical services tailored to the needs of patients while aligning with the objectives of the Pharmacy Department and the broader Health System.
Delivers comprehensive pharmaceutical care through adherence to established protocols and guidelines, while customizing medication therapies to ensure safe, effective, and economically viable patient care.
Acts as a mentor and resource for junior pharmacists, pharmacy residents, fellows, and pharmacy students.Contributes to the continuous education and professional development of pharmacy staff and other healthcare professionals.
Key Responsibilities:
- Collaborates with healthcare professionals to tailor medication therapies that promote safe, effective, and cost-efficient patient care in accordance with best practices and evidence-based guidelines.
- Designs, implements, and assesses quality improvement initiatives and clinical pathways related to medication management to guarantee optimal patient outcomes.
- Evaluates the effectiveness of clinical programs and services, implementing enhancements through systematic problem-solving methods to maximize value.
- Contributes to formulary management and collaborates with healthcare providers to ensure adherence to formulary guidelines.
- Actively engages in medical staff and service line meetings, including Pharmacy & Therapeutics Committee, to address patient care needs and service line goals.
- Takes a leadership role in service line initiatives concerning medication management and the medication use process.
- Develops, implements, and leads initiatives aimed at cost savings and cost avoidance related to medication utilization, providing regular updates on progress.
- Participates in strategic and operational planning activities, including the development of the Pharmacy Department's annual budget.
- Educates, mentors, and assesses the performance of pharmacists, pharmacy residents, and/or pharmacy students to enhance their skills in pharmaceutical care.
- Creates and delivers educational programs, training modules, and assessment tools.
- Informs other healthcare professionals about medications to promote optimal patient care through in-services, educational sessions, and direct interactions.
- Reviews, interprets, and assesses all medication orders for patients across all age groups.
- Maintains patient medication records within an automated pharmacy information system.
- Consults with and advises medical, nursing, and other healthcare professionals regarding the prescribing, dispensing, or administration of medications.
- Regularly monitors patients' total drug therapy for effectiveness, side effects, adverse reactions, toxicities, drug interactions, and incompatibilities.
- Actively participates in cardiopulmonary emergencies.
- Conducts assigned drug monitoring programs and concurrent drug use evaluation activities as required.
- Performs and documents pharmacist intervention activities, including recommendations on drug selection, dosage adjustments, and appropriate drug therapy.
- Obtains medication histories for selected patients and communicates relevant information to the appropriate healthcare provider.
- Facilitates communication with physicians, nurses, and other healthcare professionals as necessary.
- Prepares and/or verifies the accurate preparation of medications for dispensing, adhering to required techniques.
Qualifications:
Required Education and Experience
Registered Pharmacist with the relevant state board of pharmacy in good standing.
Aseptic Technique – Completion of an ACPE approved 20-hour sterile products course within the initial period of employment.
Completion of a Post Graduate Year 1 (PGY1) Residency or a minimum of three years of relevant experience.
