Travel Nurse/Home Health Care RN

Job Summary:
We are seeking a skilled Travel Nurse/Home Health Care RN to join our team. As a Travel Nurse/Home Health Care RN, you will provide high-quality patient care in a home health setting, working closely with patients, families, and healthcare teams to achieve optimal health outcomes.
Responsibilities:
- Conduct thorough nursing assessments and develop individualized plans of care in collaboration with patients, families, and healthcare teams.
- Implement and evaluate plans of care, ensuring timely and effective interventions to address patient needs.
- Provide skilled nursing services, including medication administration, wound care, and therapeutic treatments, as ordered and indicated.
- Collaborate with patients, families, and healthcare teams to promote patient independence and self-care.
- Participate in health promotion and disease prevention activities, and initiate preventive and rehabilitative nursing procedures as needed.
- Coordinate services and scheduling with the Clinical Team Manager (CTM) and Clinical Team Coordinator (CTC) to ensure seamless care delivery.
Requirements:
- Graduate of an accredited school of nursing.
- ~ 1+ years home health care or related environment experience.
- Knowledgeable in Microsoft Office applications, including Outlook, Word, and Excel.
- Ability to read, analyze, and interpret information in medical records and health plan documents.
- Access to reliable transportation to travel to client and/or patient sites within a designated area.
- Tuberculosis screening and proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease.
Preferred Qualifications:
- Bilingual (Spanish).
