We are seeking a highly skilled and compassionate Family Medicine Physician to join our team at HEALing Community Center. As a primary care specialist, you will be responsible for providing comprehensive healthcare services to patients, including ordering laboratory and radiology tests, referring patients to specialists when necessary, and analyzing medical data to diagnose conditions and propose treatment.
Key Responsibilities- Provide Comprehensive Primary Healthcare Services
- Order laboratory and radiology tests as needed
- Refer patients to specialists when necessary
- Analyze medical data to diagnose conditions and propose treatment
- Expand Community Outreach and Education Services
- Assist with planning, organizing, and implementing medical outreach events and activities within the community
- Develop and consistently update the tracking process of logged individual patient and community contacts
- Support the Expansion of the Health Services Program
- Collect engagement data and submit regular monthly reports on patients served and community connections
- Work with primary care providers and the Medical Director to improve health services for children and pregnant women
- Link Patients to Community Resources
- Cultivate relationships with referral sources and a multi-disciplinary team
- Assume quality of care for all patients of HEALing Community Center
- Adhere to High Standards of Medical Practice
- Adhere to the highest standard of medical practices, ethics, and professionalism at all times
- Assist the Medical Director in establishing medical policies and quality improvement procedures
- Education and Experience
- M.D. from an accredited institution
- Successful completion of an accredited Family Medicine Residency with preferably 2 years of clinical practice experience
- Current American Board of Family Medicine (ABFM) certification or be board-eligible
- Knowledge and Skills
- Knowledge of the Spanish language is helpful
- Familiar with FQHC is preferred
- Ability to relate to the public regardless of ethnicity, religion, and economic status
- Diagnose and Treat Patient Health Problems
- Diagnose and treat problems with a patient's health
- Provide instruction and education on all entities of medicine that affect a patient's overall health
- Perform Examination and Formulate Treatment Plans
- Perform examination of patients, formulation of differential diagnostic plans, define and develop appropriate treatment plans
- Keep Accurate Patient Records
- Keeping accurate patient records in EHR
- Stay Current with Continuing Education
- Keeping current with organized medicine and continuing education practices and CMEs
- Thorough and Methodical Approach to Patient-Centered Care
