About the Role:
River Falls Post Acute, a leading provider of skilled nursing services, is seeking a highly skilled Medical Director of Resident Care to join our team. As a key member of our Inter-Disciplinary Team (IDT), you will be responsible for assessing resident care needs, directing and supervising staff to meet those needs, and coordinating with other team members to develop and implement individualized plans of care.
Key Responsibilities:
- Assess resident care needs and develop individualized plans of care in collaboration with the IDT
- Direct and supervise staff to ensure high-quality resident care and services
- Coordinate with other team members to ensure seamless care transitions and effective communication
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Collaborate with the Administrator to develop and implement policies and procedures that promote resident-centered care
Requirements:
- Current, unencumbered license to practice as a nurse in South Carolina
- Current CPR certification
- Experience with PCC preferred
- MDS experience within the last 2 years in Skilled Nursing is ideal
