Job Summary:

The primary role of the LPN is to provide clinical support, under the direction of the clinical supervisors and/or RN professional, to ensure smooth patient flow and physician and patient satisfaction.

As a liaison between patients and providers, the LPN administers medications and treatments as per RN orders and completes all assigned tasks. Essential Generic Job Functions include:

  • Demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies.
Following the plan of care as directed by the RN and/or clinical supervisors, the LPN:

  • Complies with appropriate physician orders.
  • Observes, reports, and assists in meeting patient needs as per plan of care.
  • Records vital signs, intake, and output, and other related responsibilities.
  • Supports and reinforces patient teaching and discharge planning.
  • Assists patients with personal hygiene, nutrition, and daily activities.
  • Appropriately utilizes and cares for technical equipment and supplies.
  • Assists physicians with medical treatments, exams, and surgical procedures appropriate to the unit.
  • Reports timely and accurate changes in the patient's condition, including a change in vital signs and/or blood sugars.
The LPN turns in assignment sheets for evaluation and adapts work schedules to meet the needs of the unit. They ensure adequate staffing needs are met through willingness to float and participate in emergent situations. The LPN assists with and reassures patients and families, completing patient-related activities within scheduled time periods. They demonstrate knowledge and skills necessary to provide care, based on physical, psychosocial, education, safety, and related criteria, appropriate to the age of the patients served in assigned service areas. The LPN utilizes data information systems appropriately and adheres to HMH Organizational competencies and standards of behavior. Other duties and/or projects may be assigned.
