Physical Therapist

As a Physical Therapist with PeaceHealth Home Health, you will play a vital role in helping our patients achieve their goals and improve their quality of life. You will work collaboratively with our multidisciplinary team to provide comprehensive care to patients in their own homes.
Responsibilities:
- Evaluate patients and establish treatment plans and goals, using data from various sources, such as patient records, interviews, observation, and team members.
- Provide patient treatment according to the plan of care and modify the plan to meet patient needs, and update the plan as necessary.
- Complete documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
- Collaborate with other healthcare professionals as necessary to ensure smooth continuous care of the patients.
- Act as a liaison with nursing, medical, and other therapy staffs to facilitate problem-solving and coordination of other services and act as an educational resource.
- Make recommendations for relative content and scope of therapy services and implement policies and procedures that guide and support the provision of services.
- Recommend and develop new therapy services and programs in collaboration with staff.
- May spend minimal time to assist with evaluating and orienting new caregivers and making recommendations to management regarding equipment and space needs related to physical therapy functions.
- May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.
- Recommends and participates in development of new services and programs, quality improvement and monitoring activities.
- May participate in staff meetings, patient care meetings, educational in-services, caregiver orientation, and other professional activities.
Requirements:
- Bachelor's, Master's, or Doctorate Degree Required: Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted).
- Physical Therapist license State of Oregon.
- Requires one year of Physical Therapist experience, with home health/geriatric experience preferred.
- Basic Life Support through American Red Cross or American Heart Association.
- Valid driver's license and proof of auto insurance.
