Job Title: Advanced Practice Infectious Disease Provider

At AbsoluteCare, Inc., we are seeking a highly skilled Advanced Practice Infectious Disease Provider to join our team. As a key member of our care team, you will provide comprehensive care to patients with HIV and other chronic infectious diseases.

Job Summary:

This role is responsible for providing patient consultation, primary management of people living with HIV and other chronic infectious diseases, and developing diagnostic and treatment care plans. You will work closely with a large, integrated care team to provide whole-person care in the outpatient setting.

Key Responsibilities:
  • Provide care for patients with HIV and associated medical conditions, as well as those with other chronic infectious diseases.
  • Develop and implement comprehensive care plans for patient panels, including management of chronic diseases directly or through collaboration with primary care physicians and specialists.
  • Work in a team environment to process feedback, facilitate open discussion, and implement plans of care in collaboration with patients, their families, and other team members.
  • Develop strong transitional care plans and track patients along the continuum of care, including hospital discharges, ER visits, skilled nursing facilities, and home care.
  • Assist with educating patients regarding their chronic diseases and healthy choices, and facilitate discussions about chronic disease progress and advanced directives.
  • Practice evidence-based medicine in accordance with the PCMH model of care.
  • Assess, plan, and coordinate care for HIV/AIDS patients, and work with and lead a team to process feedback and implement plans of care.
Requirements:
  • Unrestricted Maryland Advance Practice License
  • Credentialed by AAHIVM
  • Minimum of 2 years' experience practicing as an Advance Practice Provider
  • BLS
  • DEA
  • Experience in population health management is preferred
  • Experience in working with psychosocially and medically complex patients is strongly preferred
  • Knowledge or experience with NCQA PCMH model is preferred
