Job Title: In-Home Care RN/LVN

Family Tree Private Care is a leading provider of concierge-level advisory services to older adults, those living with disabilities, and their families. We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our team as an In-Home Care RN/LVN.

Job Summary

We are looking for a dedicated and caring professional to provide high-quality care to our patients in the comfort of their own homes. As an In-Home Care RN/LVN, you will be responsible for assessing and charting patient observations, completing evaluation tasks, and educating caregivers and family members on patient care. Responsibilities
  • Travel to assigned patients' homes and manage their care plan according to physician's instructions.
  • Assess and chart observations of the patient's condition at each visit.
  • Complete evaluation tasks, including reviewing medication and vital signs.
  • Dress or redress wounds and assess the healing progress.
  • Educate caregivers and family on the aftercare or ongoing care of the patient.
  • Keep the patient comfortable through palliative care.
  • Recommend tools or devices that may improve the quality of life for the patient.
Requirements
  • Strong interpersonal skills that allow you to build personal rapport and establish trust with patients.
  • Initiative to make independent decisions when required and accept direction and requests from patients.
  • The ability to follow instructions but also act independently if necessary.
  • Excellent observational and problem-solving skills.
  • Ability to act swiftly in emergencies and work efficiently.
  • Flexibility and patience to work in a variety of living environments.
  • Knowledge of clinical skills including changing bandages, giving tube feedings, administering shots, and setting up and monitoring IVs.
Qualifications
  • Relevant license and certification.
  • A valid driver's license and reliable transportation.
  • Strong candidates have a minimum of 1 year of experience working in the home care industry (or related industry - hospice, home health, hospital, etc.).
  • CPR Card.
