Job Title: Licensed Practical Nurse

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team at BrightStar Care of Cincinnati N & E. The ideal candidate will have a strong background in managing complex, skilled cases, including but not limited to trach/vent care and advanced wound care.

Responsibilities:
  • Provide high-quality, compassionate care to pediatric patients in a home setting.
  • Manage and monitor complex medical equipment, including tracheostomy tubes and ventilators.
  • Perform advanced wound care and treatment procedures as required.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Educate families on care procedures, equipment use, and health management.
  • Ensure accurate and timely documentation of patient care and progress.
Qualifications:
  • Valid LPN license in Ohio.
  • Minimum of 1 year of experience in home health care, with a focus on pediatrics. Preferred 3 years.
  • Proven experience with complex cases such as trach/vent care and wound management.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • CPR certification required, and any additional relevant certifications are a plus.
  • Ability to work independently and manage time effectively in a home health setting.
