Job Summary

We are seeking a skilled and compassionate Home Health Nurse Specialist to join our team of dedicated clinicians. As a Home Health Nurse Specialist, you will play a vital role in delivering high-quality care to patients in their homes, skilled facilities, and through telehealth visits. You will work collaboratively with our multidisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient. Your expertise and compassion will make a significant difference in the lives of our patients and their families.



Key Responsibilities


  • Conduct comprehensive assessments of patients' health status, including medical history, medication adherence, and social environment
  • Develop and implement personalized care plans that address patients' physical, emotional, and social needs
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle practices
  • Collaborate with our multidisciplinary team to coordinate care and ensure seamless transitions between settings
  • Document patient information and care plans in a timely and accurate manner
  • Participate in quality improvement initiatives to enhance the quality of care and patient outcomes


Requirements


  • Master's degree in nursing or a related field required
  • Current RN licensure and NP certification in a relevant specialty
  • Board certification by a recognized professional organization
  • Minimum of 1 year of experience as a nurse practitioner in a home health or primary care setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team


Compensation and Benefits

We offer a competitive salary of $90.00 to $95.00 per completed assessment, depending on county. Our benefits package includes a mileage reimbursement plan, and we offer opportunities for professional growth and development.
